I had the same problem and to be honest I still haven't figured out what I did to correct it. I know that if I use the 16 bit driver I'm okay. Have you tried some of your extensions? I think I had a problem with the superdisk driver. I say create a set of minimal extensions zap your pram and try again. Good luck, keep me posted as to your solution.
